Designed to improve access to higher quality care, we are now scheduling 90 minute appointments. Use the link below if you would like to be contacted.

Asthma Group Sign Up Form.

What is a group asthma visit? It is a time where 5 to 10 children and their caregivers come together to share and learn from our providers and staff. We encourage asking questions and sharing experiences and information. You will learn from our healthcare team and we will learn from you. During the group visit, each child will have the opportunity to go to a private area for individual care as needed. The healthcare team will typically have a Nurse Practitioner or Physician, one or more Registered Nurses and other support staff as needed.

Why sign up for a group asthma visit? Group visits are rated highly in terms of patient/family satisfaction for several reasons. It provides an opportunity to relate to other people dealing with similar health issues, plus receive reliable information from the healthcare team. Group members develop a bond with each other and the healthcare team members. Members serve as role models for each other. Each family signs a confidentiality agreement to assure a private and secure environment to share health information.

Group asthma visits are NOT meant to take the place of regularly scheduled visits with your child’s healthcare provider. Nor are they meant to manage an acute flare-up of asthma or other health issue.
